GOAL:

To provide enhanced services by linking, monitoring and advocating, in order to ensure that an individual with mental illness, quality of life may be maintained or improved in the least restrictive environment, keeping in mind continuum of care.

Referrals are received from therapists, hospitals, family members and other agencies.


SPOA:

Single Point of Access (SPOA) is not a service, but a committee of representatives that review referral information in order to assist individuals with referrals to services that can assist adults in meeting their needs. 

The SPOA Committee consists of representatives from:

  • Social Services
  • Probation 
  • Council on Addiction Recovery Services 
  • Day Treatment 
  • Department of Aging
  • Community Action 
  • Catholic Charities 
  • Intandem 
  • Behavioral Health Units 
  • as well as any agency that may be involved with a SPOA referral.

Referrals may be provided to services that can help with:

  • Housing assistance
  • Educational/Vocational programs
  • Physical Health
  • Mental Health
  • Self Care
  • Economic
  • Transportation
  • Skills building
  • Substance Abuse

Adult SPOA Meetings are on the First and Third Tuesday of Each Month, starting at 10:30AM
