Care Coordination provides three levels of service:
- Intensive for children and adults, Supportive for children and adults, and regular Case Management for adults.
- AOT case management is available for persons who are considered high risk under Kendra's Law and may be court ordered to receive services.
- Case Management also provides forensic case management to persons with mental illness who are incarcerated in the County jail.
The components of Care Coordination are: linking, advocacy and monitoring to all service providers and support systems so that an individual's quality of life may be maintained and improved.
