We accept the following: Medicare, Medicaid, Blue Cross/Blue Shield, Independent Health, Molina, Fidelis, Univera, and Medicare-type insurances. Cash cost = $25

COVID-19 Vaccine Insurance / No Cost
We accept the following: Medicare, Medicaid, Blue Cross/Blue Shield, Independent Health, Molina, Fidelis, Univera, and Medicare-type insurances.
There is no cash cost for the uninsured or underinsured; the new 2023-2024 COVID-19 Vaccines will be provided through Vaccines For Children (VFC) or Bridge Access Program. If you are uninsured or underinsured: please complete the registration process above and contact CCHD at 716-701-3439 to discuss eligibility for these programs.
